In a comprehensive study published in The Lancet Global Health, it has been revealed that a staggering 40 million women worldwide are likely to endure persistent health challenges stemming from childbirth. The findings, part of a special series on maternal health, expose a significant gap in addressing postnatal conditions that extend beyond the initial six weeks postpartum.

The study, encompassing approximately 140 million new mothers annually, challenges the misconception that the majority of women do not face morbidity after childbirth. Even those with uncomplicated vaginal births without interventions are not immune to postpartum complications, as highlighted in the report.

The persistent conditions highlighted in the research comprise dyspareunia, impacting 35 percent of postpartum women, low back pain at 32 percent, anal incontinence at 19 percent, and urinary incontinence ranging from 8 to 31 percent. Additionally, anxiety is reported in 9 to 24 percent of cases, depression in 11 to 17 percent, perineal pain in 11 percent, fear of childbirth (tokophobia) at 6 to 15 percent, and secondary infertility at 11 percent.

One critical aspect emphasised by the authors is the necessity of public healthcare systems addressing these conditions, given that they often persist even after postnatal services have been availed. The study underscores the importance of effective care throughout pregnancy and childbirth as a preventive measure to detect risks and avert complications post-birth.

The report raises concerns about the global neglect of postpartum complications in clinical research, practice, and policy. The authors discovered a lack of recent high-quality guidelines for treating 40 percent of the 32 priority conditions analysed in their study. This absence of guidelines and data points to a critical gap in understanding and managing these postnatal health issues.

The Lancet authors call for urgent attention to be directed towards these often-overlooked postpartum complications. The study advocates for increased and improved measurement of these conditions, especially in lower-middle-income countries where the scale of the burden remains inadequately understood. The hope is that this comprehensive exploration of postpartum health issues will pave the way for improved clinical strategies, policies, and research initiatives to better support the millions of women facing long-term health challenges after childbirth.
