What is your first reaction when you see a dry scaly patch on your skin that may cause an uncomfortable itching? Or, if the rash is on the scalp, do you assume it is dandruff and use an anti-dandruff shampoo? What if these 'remedies' don't work? In some cases, scaly skin rashes on the arms, legs, and scalp may signal that it is psoriasis.

Dr Abir Saraswat, Consultant Dermatologist, Indushree Skin Clinic, Lucknow, explained psoriasis, its effects, and the importance of early detection.

What Is Psoriasis

psoriasis

Psoriasis is a persistent autoimmune disorder that mostly affects the skin. It is characterised by the rapid buildup of skin cells, leading to the formation of thick, scaly, red patches known as plaques. These plaques often appear on the elbows, knees, scalp, lower back, and other areas of the body. Psoriasis is not contagious, and its exact cause is still unknown. However, it is believed to involve a combination of genetic, immune system, and environmental factors.

Impact Of Psoriasis

Dr Saraswat said that delay in seeking medical attention for psoriasis often leads to worsening symptoms.

Itchy and scaly patches can interfere with sleep and daily activities, while the appearance of these patches can affect a person's social behaviour.

Psoriasis can also affect the joints, causing swelling and pain, a condition known as psoriatic arthritis.

It is associated with systemic inflammation, affecting not only the skin and joints but also other organs. This chronic inflammation increases the risk of heart disease, diabetes, liver disease, and inflammatory bowel diseases.

Dr Saraswat added that the specific cause for the systemic inflammation that occurs in psoriasis is still unknown. However, there is some evidence that it could be a genetic propensity. Skin diseases, injuries, extreme weather, and stress have also been evaluated since they can produce systemic inflammation.

Why Is Early Diagnosis Important?

Dr Saraswat highlighted that psoriasis has no complete cure, but early diagnosis allows timely intervention and management. Current treatments can provide relief from skin rash and joint pain for extended periods, to achieve long-term clearance of patches. Early treatment can alleviate the emotional burden of psoriasis, as severe cases often lead to depression and feelings of isolation. Also, early diagnosis enables dermatologists to monitor overall health, refer patients to specialists if necessary, and implement lifestyle modifications to improve outcomes.

Advancements In Treatment

Dr Saraswat said that if therapy is started on time, patients can experience complete skin clearance. This reduces the emotional burden of psoriasis patients. Newer therapies, such as biologics, have revolutionised the treatment of psoriasis. Timely use of these treatments can decrease the risk of heart and liver disease associated with psoriasis. Combined with lifestyle modifications, early treatment brings quick relief from pain, itching, and skin patches, providing long-lasting benefits.

Bottomline

Early diagnosis helps dermatologists to monitor a patient’s overall health and refer them to other specialists if required to prevent damage to internal organs. Patients can be advised to improve their lifestyle, keep their weight in check and quit smoking. If these lifestyle modifications are made early, they act as a support to the treatment and help to keep the inflammation under control. Early treatment, particularly with the availability of new therapies now brings quicker relief from pain and itches and clears up the skin patches for a longer period.
